- At most two (2) moving violations while driving a personal or commercial vehicle within the last three (3) years.
- There have been no serious traffic violations while driving a commercial vehicle in the last three (3) years.
- No DUI, DWI, OUI, or reckless driving involving alcohol/drugs convictions in the last ten (10) years.
- There have been no preventable accidents while driving a commercial vehicle in the last three (3) years.
- No accident resulting in getting treatment at a hospital or any medical center within the last 10 years that could’ve been prevented, while driving a commercial vehicle.
- No preventable accidents result in a fatality or catastrophic injury in driving history (commercial motor vehicle).
